The card also offers designated blocks to fill in the employee's name, date, job name or numbers, kind of work, hours, rate, amount, total regular time and total overtime. At the bottom, boxes are labeled, Approved By, Date Paid, Check No., Withhold, F.I.C.A., Medicare, State W.H., Total Earnings, Total Deductions, and Net Pay.
